If the login attempt fails the system will now inform the user if their account is locked (due to too many failed logins), if the user's password is expired and needs to be reset, or if the username/password combination was incorrect.
Additionally, the system will now properly trigger incorrect password attempts and users will receive 5 attempts to log in. |

When adding a therapy without a corresponding physician (e.g. adding surgery therapy without a surgeon in case data), only throw a warning for cases diagnosed in years 2013 and later. |
